Please note an important change to the waiver form requirements: It is now required that EVERY student have a permission form for EVERY event. Any previous forms you've completed are no longer valid. Please remember to complete a form for every event your child participates in. We apologize for any inconvenience. Thank You!
LifePoint Church requires a waiver form be completed for every student participating in a UGR special event. This form is both a medical and a liability release. It must be signed by a parent/guardian. Click here for a WAIVER FORM.
